Client Forms

When you schedule your first appointment, we will email you an invitation to create an account in the client portal. Before your first session, please login to the client portal and complete your profile, demographic information, intake forms, billing information (required), and insurance information (if utilizing).  This will take about 15 minutes and saves us time in the first session to focus on what’s bringing you to counseling.  Please be aware that completion of the client portal is required prior to meeting so if you have any questions or problems accessing the portal or completing this process, please contact us prior to your first appointment.
